Operation Headwaters was a Group I undercover operation run by the Federal Bureau of Investigation, the U.S. Department of Health and Human Services Office of Inspector General, and the U.S. Postal Inspection Service under the guidance of the U.S. Attorney's Office for the Southern District of Illinois and the U.S. Department of Justice. 

The Operation Headwaters Team has posted criminal collections for FY 2004 in the amount of $205,249,916. In addition, the team has recovered $191,258,088 for the Medicare Trust Fund and has been the force behind recoveries to state Medicaid Fund Control Units of another $32,046,662. These extraordinary collections were the result of the aggressive investigation, prosecution, and case management by the entire Operation Headwaters Team.
