Exhibit 5: Applications Received by ITVERP.

This bar graph shows the number of applications received during the past seven reporting periods. From September 2006 to August 2007, 24 applications were received; from September 2007 to August 2008, 18 applications were received; from September 2008 to August 2009, 55 applications were received; from September 2009 to August 2010, 90 applications were received; from September 2010 to August 2011, 39 applications were received; from September 2011 to September 2012, 36 applications were received; from October 2012 to September 2013, 38 applications were received. Note: the period for September 2011 to September 2012 covers 13 months.

Exhibit 6: Status of ITVERP Claims (as of September 2013).

This pie chart shows the status of all 300 ITVERP claims, as of September 2013 — 21 claims were active, 200 claims were paid, 50 claims were denied, and 29 claims were declared claimant unresponsive.

Exhibit 7: Average Length of Time To Process ITVERP Claims.

This table shows the average length of time to process claims during the reporting period, based on the status of the claims. The 28 paid claims* took an average of 266 days. The 11 denied claims took an average of 141 days.

*Note regarding the number of claims: One paid claim took a total processing time of 1,136 days; however, the claim was inactive for 1,106 of those days and, as a result, is not included above.

Exhibit 8: Average Time Paid Claims Spent in Each Step of the Claims Processing.

This bar graph shows the average length of time required to complete each step in the application process for paid claims. 99 days of the processing time was used to complete the application (claimant), 104 days was used to determine eligibility (FBI, NSD), 88 days of processing time was used to verify expenses (ITVERP and providers), and 91 days of processing time was used to review recommendation memo* (OJP).

Exhibit 9: Average Time Denied Claims Spent in Each Step of the Claims Process.

This bar graph shows the average length of time required to complete each step in the application process for denied claims. It took an average of 60 days to review claims and clarify policy implications (ITVERP, OVC, OGC), 87 days was used for incident designation (NSD), and 81 days spent reviewing recommendation memos* (OJP).

Exhibit 10: Reimbursement Requests, by Expense Category.

This bar graph shows the number of reimbursements requested, by category. During the reporting period, there were $65,526 in 13 reimbursement requests for medical expenses, $48,930 in 15 reimbursement requests for mental health expenses, $867,456 in 20 reimbursement requests for property loss expenses, $70,406 in 8 reimbursement requests for funeral and burial expenses, and $97,862 in 19 reimbursement requests for miscellaneous expenses.

Exhibit 11: Reimbursements Paid, by Expense Categories.

This bar graph shows the total amount of all reimbursements paid to each expense category. $8,531 in 7 claim reimbursements were for medical expenses; $9,231 in 7 claims reimbursements were for mental health expenses; $24,483 in 13 claim reimbursements were for property loss expenses; $62,587 in 10 claim reimbursement were for funeral and burial expenses; and $18,352 in 15 claim reimbursement were for miscellaneous expenses.

Exhibit 12: Estimated Future Liability of ITVERP.

This bar graph shows ITVERP’s estimated future liability, by type of reimbursement. ITVERP’s estimated future liability comprises $34,775 in medical expenses, $14,002 in mental health expenses, $31,704 in property loss expenses, $57,254 in funeral and burial expenses, and $26,854 in miscellaneous expenses.
